Important: Please refrain from wearing any scented lotions or perfumes on the day of your exam.

What to bring with you to your exam:

1. Insurance Card

Please bring your Driver's License or other form of ID along with your insurance card to your appointment. Without a photo ID we will not be able to perform the exam.

2. Medical History and Registration forms

You will need to print out and complete the following forms and take them with you to your exam.  These forms are also available as links in the confirmation email. If you are unable to open the link or print the forms, please call 206-320-2885 for assistance.
